How to Apply for Access-a-Ride

There are 3 components to the Access-a-Ride application:

1. A customer application

2. A medical verification form to be completed by the licensed professional most knowledgeable about your condition

3. An In-person Interview & Transportation Assessment *

*RTD can provide transportation to and from this appointment at no cost.

The completed application and medical verification forms may be faxed to 303-299-2169 OR mailed to (please make sure that pages printed double sided are faxed double sided):

Once RTD has reviewed the completed documents you will be contacted to schedule the in-person assessment. If it has been two weeks since the application was submitted and you have not heard from us, please feel free to contact us at 303-299-2960.

Partial applications that are still incomplete after 90 days of submission will be withdrawn.

Important note: All determinations are made based on the information gathered from your application, medical verification form, and your assessment.

Applicants who wish to appeal their eligibility determination will be provided with information on how to make an appeal when their assessment results are mailed.
